Before your first visit to MSAC, we encourage you to complete this online questionnaire, that aims to better understand your symptoms, concerns, general medical background and the reasons for your referral and visit. It will also ensure that your visit with us is more efficient and targeted towards what impacts you most.

Outcome Measures
At MSAC, a key aspect of assessing your problem is using standardised international questionnaires, to understand how certain conditions impact your quality of life and function. You will be asked to regularly repeat these questionnaires throughout your care with us to further assess your progress. You will be directed by MSAC staff as to which PROMs we would like you to complete.

These sheets contain advice on common symptoms, causes, and treatments for these conditions, and are designed to augment what is discussed with you during your consultation with the MSAC team.
